The Millennium Park Memorial Dedication Program provides an opportunity for individuals, organizations or service groups to make donations towards enhancing Millennium Park and public spaces through a Memorial Dedication.

Your dedication can be made by placing a plaque on a park bench, picnic table, bike rack, dog pet stand, in front of a tree, or other park improvement projects for Millennium Park. Many of the amenities located in our parks today were previously sponsored through the generosity of individuals or organizations.
The first step in making an application is reviewing the Millennium Park Memorial Dedication Program Information Brochure located on our website, choosing an item that you wish to dedicate, and completing and returning the Program Donation Form.
